Become a Member Agency
We invite hospice agencies from throughout the United States
to join with us in helping
hospice patients and their families
during the difficulties of terminal illness.

When you become a Member Agency, your Agency may apply
to the Foundation for financial grants for services and products
not covered by Medicare, Medicaid, or private insurance. At your
request, we will provide you with an Immediate Use Fund you
may use to secure personal items for patients without going
through the grant process. Only a Member Agency may apply
for grants and have an Immediate Use Fund, according to the
guidelines in our
Policies and Procedures Manual.

How to Apply
Print and complete the one-page Member Agency Application
Form and have your Agency Director sign it and send it to the
Foundation. Keep a copy for your records.

The Foundation Board of Directors will notify the Agency Director
within thirty (30) days of receiving the Application whether the
Agency has been approved as a Member Agency.
