About Us
CHA Foundation was established as a not-for-profit corporation
in 2001 and is governed by a volunteer board of directors.

The Foundation works with hospice agencies to help provide
physical, social, spiritual and emotional support to terminally ill
patients and their families.

Contributions
Contributions may be made to a local hospice agency or
directly to the Foundation and will be used for patient care,
family support, volunteer enhancement, and community
education. The Foundation prepares and mails thank-you
letters to donors and notifies families and caregivers of
memorial gifts.

Grants
When an agency recognizes a patient's need not covered by
Medicare, Medicaid, or private insurance, the agency director
may submit a grant request to the Foundation. The request is
carefully reviewed by the Foundation board of directors for its
appropriateness to patient care, family support, volunteer
enhancement, or community education. Funds for approved
requests are distributed when appropriate receipts are received
at the Foundation.
